Planning for your death ensures that your wishes are honored and that your loved ones are not left guessing and making difficult decisions on your behalf at a time that emotions are often heightened. I am available to support this process at any stage. It can be particularly emotional if you have a life limiting diagnosis.  I can help you navigate potentially difficult conversations with loved ones when there are lots of strong emotions and offer you some peace of mind.

We will complete an assessment of your current plan and ensure all the legal details are in order for managing your estate after you pass. We will also ensure that your comprehensive wishes for end of life care are clearly documented for your loved ones.

  • Complete an assessment of your current end of life plan.
  • Provide vetted referral for legal services as needed.
  • Build a detailed road map of your accounts. 
  • Update beneficiaries.
  • Walk you through end of life care plans and wishes. Creating a document for you to share.
  • Provide referrals, including scheduling, for mental health supports as needed.  
  • Outline wishes for how your remains will be handled, including details about a memorial or funeral. This can be as detailed as feels right for you. 
  • Plan for any legacy wishes.

EMOTIONAL SUPPORT
FACILITATED CONVERSATIONS

Two individual sessions in which we identify emotional goals for end of life and make a plan for sharing those wishes with your loved ones. Followed by one facilitated group conversation to support those goals being understood by your support system. For some families acknowledging that there is even a reason to make end of life plans can be difficult.

  • Help you identify what “cleaning up” both physically in your home and in your relationships you would like to do and prioritize, and make a plan on how to do that. 
  • ​Facilitate end of life loved ones meeting to ensure that the wishes for end of life care, funeral arrangements and/ or legacy wishes are known. Providing a space and structure to go through the details in a caring and guided conversation. 
  • This includes a one on one meeting to plan for the group facilitated discussion, followed by guided conversation with loved ones. These meetings can either be in person or conducted remotely depending on the needs of the group.
  • A written copy of the plan will be provided.

PROJECT MANAGEMENT
PRACTICAL SUPPORT 

When someone we love passes there are numerous practical details to be managed. Beyond the funeral, there is executing the details of the will, orchestrating closing of accounts, paying bills, and contacting and applying for appropriate benefits. Managing the logistics of the death of a loved one can be overwhelming.

  • I will meet with you to create a detailed plan of the tasks that are to be completed. This will be tailored for your needs but some examples include:
  • Closing digital and business accounts
  • Hiring and managing professionals for house packing, cleaning, or preparing a home for sale.
  • Providing referrals for grief supports (including researching in network options for your insurance)
  • Filing with social security,
  • Manage life insurance policies, 
  • Coordinate with estate attorney regarding probate.

As care needs increase it is often the case that more arrangements are necessary to meet the family’s needs. I am available to help make a concrete care plan when a new care arrangement is essential.

  • We will meet to complete an assessment and create a care plan with concrete goals. Some examples of these services could be:
  • Colloborate and coordinate providers
  • Provide referrals, including scheduling, and utilizing your insurance in-network providers as possible
  • Research and assist in hiring in home caregivers to provide respite or as much care as is needed
  • Create a transition plan for moving into more supportive housing
  • Coordinate and support downsizing a family home
  • Evaluate and assist in decision making about next care options
  • I provide a written plan and will collaborate with your friends and loved ones if you wish
